TKKW Life Insurance
- This Insurance can be browsed on ATOM Store App and ATOM website by all ATOM users.
- Data usage fees for browsing the program by using internet will be charged from the data package bought by user or will be deducted from the primary remaining balance with PAY-AS-YOU-GO-RATE.
- ATOM Myanmar reserves the right to withdraw or change this offer at any time. Customers are required to comply with the rules established by insurance companies regarding the product purchase.
- ATOM Myanmar shall not have an access right to your personal information, documentation, claim procedures for insurance application process. Only insurance companies are the sole entity that collect and know your personal information for the application process.
- ATOM shall not be responsible for any personal information provided by the own consent of the applicant to insurance companies for purchasing insurance.
- ATOM shall have no liability for the misuse of the insurance service by the user customers or partner.
- Any dispute relating to the purchasing of insurance shall be settled between the insurance companies and customers.
- During the validity period of TKKW policy, the following benefits shall be provided subject to the terms and conditions herein:
- A compensation amount shall be payable upon the death of the insured person during the coverage period.
- A compensation amount shall be payable if the insured person becomes totally and permanently disabled during the coverage period.
- In the event of accidental death, total and permanent disability, or bodily injury sustained by the insured person due to an unforeseen incident, compensation shall be provided in accordance with the policy terms.
Benefits and Entitlements
- The following benefits shall be payable under this policy, subject to the terms and conditions:
- In the event of the insured person’s death or total and permanent disability, a compensation amount equivalent to 100% of the sum insured shall be payable.
- If the insured person dies or becomes totally and permanently disabled due to an accident, a compensation amount equivalent to 200% of the sum insured shall be payable.
- In the event of any of the following injuries resulting from an accident, a compensation amount equivalent to 50% of the sum insured shall be payable:
- Total and permanent loss of hearing in both ears
- Permanent loss of speech
- Loss of one eye
- Loss of one limb (arm or leg)
- Surgical Injuries Due to Accident
- In the event of any of the following conditions resulting from an accident, requiring surgical intervention, a compensation amount equivalent to 15% of the sum insured shall be payable:
- Skull fracture requiring craniotomy (surgical opening of the skull)
- Vertebral fracture requiring surgical treatment
Exclusions – Circumstances Not Covered
The insurer shall not be liable to pay any benefits under this policy for death, total and permanent disability, or bodily injury resulting directly or indirectly from the following circumstances:
- Any incident arising from the insured person committing or attempting to commit a criminal offense.
- Participation in war, carrying weapons of war, or involvement in any war-related activities.
- Participation in high-risk or extreme sports and adventurous activities.
- Involvement in riots, civil commotion, public demonstrations, strikes, acts of terrorism, or attacks by armed groups, whether the insured person is actively involved or not.
- Death or disability resulting from self-inflicted injury occurring within two (2) year from the policy issue date or reinstatement date.
- Death or disability caused by any pre-existing condition that was not disclosed at the time of policy application.
- Any condition that was excluded under a new offer made at the time of policy issuance but not accepted or disclosed by the insured.
- Illnesses resulting from infection with Human Immunodeficiency Virus (HIV) or Acquired Immunodeficiency Syndrome (AIDS).
Claims Procedure
- In the event of an incident covered under this policy, the following procedures must be followed
Notification of Claim
- A written notice of claim must be submitted to us within thirty (30) days from the date of the incident.
Claim Forms
- Upon receipt of the written notice, we will issue the necessary claim forms within fifteen (15) days for the claimant to complete and submit supporting documents.
Submission Deadline
- The completed claim form and all required supporting documents must be submitted to us in an acceptable format and manner within ninety (90) days from the date of the incident.
Medical Examination and Autopsy
- We reserve the right to request the insured person to undergo a medical examination or any other appropriate investigation to verify the claim. Where not prohibited by law, we also reserve the right to request an autopsy.
Required Documents for Claim Submission
The following documents must be submitted to us:
- The original insurance policy document
- Proof of ownership or entitlement to claim by the claimant
- Identification and authorization documents of the insured or the authorized claimant
- A duly completed claim form and medical report
- Evidence supporting the cause of the claim under this policy
- Any additional documents we may reasonably request to process the claim
